Credit Card will not be charged for office visit &/or fees. Please bring a card or cash to pay for your appointment.

CANCELATION POLICY:

If a member of your party needs to cancel or reschedule this appointment for any reason, we require 48 hours notice. Your appointment must be cancelled by the time of your appointment on the business day prior. If you cancel with less than 48 hours notice you will be charged a missed appointment fee of $99.99 per person.
